Questions About Placement & Eligibility

The application process begins through the individual’s caseworker. If there is interest in A Sovereign Place as a provider, the caseworker will contact us directly to discuss eligibility and next steps, including scheduling a tour of the home.

To be eligible for placement at A Sovereign Place, the individual must:
Be actively connected with a Community Services Board (CSB)
Have a clinical diagnosis of a developmental or mental disability
Have an approved Medicaid Waiver for community-based living
Eligibility and placement consideration are based on the information submitted by the caseworker.

Approval is based on a review of the individual’s clinical information, support needs, and waiver status. The timeline can vary and depends largely on how quickly required documentation is submitted by the caseworker. We work collaboratively to move the process forward as smoothly as possible.

While families and guardians are encouraged to reach out with questions, applications are submitted through the assigned caseworker. Once interest is confirmed, the caseworker will coordinate communication and arrange a tour if A Sovereign Place appears to be a good fit.
